- Screening
A basic questionnaire and needs survey is completed either in person or via the
telephone. This includes general geographic information and informal information
about the services you are requesting, as well as any immediate needs you may have.
This will also help us determine what criteria you meet for services
- Assessment
After an initial screening, a comprehensive assessment, which includes a bio-psychosocial
is completed. This is a formal process by which a clinician will discuss your strengths,
needs, abilities and preferences, and come to an agreement with you about your intent
for services.
- Treatment Planning
Usually occurs during the first two to three sessions. Goals and objectives for
services are agreed upon. A written plan for discharge and community referrals is
also agreed upon. ”What do you want to work on?” “How will you know when it’s time
to leave?”
